Your item was sold 4 days ago, has the buyer paid, should it be shipped or is he picking it up?

Not paid = after at least 4 days, the purchase can be canceled due to non-payment!
Paid = according to your details, ship it; otherwise there will be a defect in the account.
For pickup = set a deadline and then cancel it and make sure the 30 days from purchase are not up, otherwise it can no longer be refunded via ebay.
And for pickup-only listings, you clearly state in the listing by when it must be picked up at the latest.
